Job Overview

Imagine Art is a 501(c) faith-based nonprofit serving artists with and without disabilities in our free, public art studio in East Austin. We also provide services to individuals with disabilities in their homes and communities throughout Travis County via two medicaid waivers (HCS and TxHmL).

Our ministry is seeking a Part Time Long Term Care Registered Nurse to share a caseload with another nurse who works M/W/F during the week.

Registered Nursing services are provided in a community bsaed setting on the schedule of Thursday, Friday, Saturday and Sunday. Start and end times are flexible, but anticipated to be 6-8 hours per day. The nurse will also be on call during the 4 day shift with expectation for 2-3 calls regarding medication inquiries.

Pay: $38.00 per hour / Expected hours: 20 per week
Location is 1127 Tillery Street.

Job Responsibilities

Duties Include:

1) Reviewing medical records and physician orders;
2) Completing in-home face-to-face assessments with each client with recommendation for regular services; with reassessments on a regular basis (at least annually);
3) Medication and nurse tasks delegation for paid unlicensed direct care staff;
4) Communicating with each clients' Service Planning Team either in-person or via phone/email to report on clients’ progress;
5) Communicating with clients, their family members or guardians and direct care staff to ensure understanding of trainings and nursing tasks;
6) Administrative tasks such as phone calls and emails;
Ability to attend Service Planning Meetings (in-person or over the phone), face-to-face home visits;
7) Understanding regulations related to Nursing within the Home and Community-Based Service Waiver;
8) Reporting to and communicating with Imagine Art administrative staff frequently via face-to-face meetings, phone calls, email, etc;

Job Qualifications

Minimum Qualifications: RN License; Clean background; Reliable transportation; Thur/Fri/Sat/Sun - weekend schedule availability; Ability to manage and report hours effectively; Experience with G-Suite (Gmail, Drive, Docs, etc) Experience with individuals with Intellectual or Developmental Disabilities

Preferred Skills -
ASL proficient
Experience with individuals dually diagnosed (IDD and Psychiatric Diagnosis)
Experience with nursing through Medicaid Waivers - HCS, TxHmL, ICF or group home settings

Additional Information

The nurse will be stationed at 1127 Tillery Street, the Live+Make apartments where 50% of the caseload resides.

These clients are persons with intellectual developmental disabilities who receive services from Imagine Art in their homes via the Home and Community Based Waiver for individuals with Intellectual/Developmental Disabilities. Nine of the clients are living in a group home setting, at the property, with 24/7 staffing, requiring RN delegation, MAR review and reconcilations of medications. In addition, nine of the clients live independently in their own apartment, at this same address and they require assessment, health training and oversight of medications and appointments. Additional clients reside off campus in their own home or adult foster care placements and may require home visits, virtual visits or phone calls to provide support and coordination. Over 90% of the position is anticiapted to occur at this single address.
